Clownfish are among the hardiest marine fish available, but they are susceptible to several diseases that are common in saltwater aquariums. Most clownfish diseases are introduced through new fish added without quarantine and are preventable. This guide covers the most common clownfish health problems, how to identify them, and what to do.

Marine Ich (Cryptocaryon irritans)

Marine ich is the most common disease in saltwater aquariums. It is caused by the ciliate parasite Cryptocaryon irritans and is distinct from freshwater ich — treatments that work for freshwater ich do not work for the marine version.

Symptoms: Small white spots on the body and fins resembling grains of salt; flashing (rubbing against rockwork or substrate); rapid breathing; clamped fins; lethargy; appetite loss. Spots may disappear temporarily as the parasite completes its life cycle, but the infection is still present.

Cause: Parasite introduced through new fish, corals, or live rock without quarantine; triggered or worsened by stress and temperature fluctuations.

Treatment: Marine ich cannot be reliably treated in a display reef tank because most effective medications harm invertebrates and coral. The standard treatment protocol is:

  1. Move affected fish to a separate quarantine or hospital tank
  2. Treat with copper-based medication (copper power or similar) at therapeutic levels, or use hyposalinity (1.009–1.010 specific gravity) for 4–6 weeks
  3. Leave the display tank fallow (fish-free) for a minimum of 72 days to break the parasite life cycle

Prevention: Quarantine all new fish for 4–6 weeks before adding to the display tank. This is the single most effective prevention measure.

Brooklynella (Brooklynella hostilis)

Brooklynella is a ciliate parasite that primarily affects clownfish and is one of the most serious diseases in the marine hobby. It progresses rapidly and can be fatal within days if untreated. It is sometimes called clownfish disease.

Symptoms: Excessive mucus production; skin peeling or sloughing; rapid, labored breathing; lethargy; loss of appetite; the fish may appear to have a cloudy or patchy appearance on the body. Progression is significantly faster than marine ich.

Cause: Ciliate parasite, most commonly introduced through wild-caught or stressed fish. Captive-bred clownfish are less susceptible but not immune.

Treatment: Brooklynella requires immediate treatment. Move affected fish to a hospital tank immediately. Formalin-based treatments are the most effective. Freshwater dips can provide temporary relief but are not a cure. Follow manufacturer instructions carefully — formalin is toxic at incorrect doses. Do not delay treatment.

Prevention: Quarantine all new fish. Choose captive-bred clownfish over wild-caught when possible.

Velvet (Amyloodinium ocellatum)

Marine velvet is caused by the dinoflagellate parasite Amyloodinium ocellatum and is one of the most dangerous diseases in the saltwater hobby. It spreads rapidly and is often fatal if not caught early. It is sometimes confused with marine ich but progresses much faster.

Symptoms: Fine gold, rust, or yellow dust-like coating on the body and fins (most visible under a flashlight at an angle); rapid breathing; flashing; lethargy; appetite loss. Fish may appear to have a velvety sheen. Gills are often affected before visible body symptoms appear.

Cause: Parasite introduced through new fish or live rock without quarantine. Highly contagious — spreads rapidly through a tank.

Treatment: Move all fish to a hospital tank immediately. Treat with copper-based medication at therapeutic levels. Chloroquine phosphate is also effective. Leave the display tank fallow for a minimum of 72 days. Velvet progresses faster than ich — do not delay treatment.

Prevention: Quarantine all new additions. Velvet can survive on live rock and corals, making it particularly difficult to eradicate from a display tank once established.

Uronema (Uronema marinum)

Uronema is caused by the ciliate protozoan parasite Uronema marinum and is more common in fish kept at lower salinity or in stressed conditions. It is an aggressive disease that can progress rapidly.

Symptoms: Red, hemorrhagic sores on the body; tissue damage and ulceration; lethargy; rapid deterioration. Unlike ich or velvet, Uronema does not produce a characteristic coating — the red sores are the primary visible symptom.

Cause: Protozoan parasite; more common in fish maintained at low salinity or under chronic stress. Can be introduced through new fish.

Treatment: Move affected fish to a hospital tank. Raise salinity to normal reef levels (1.025–1.026) if the fish was kept at lower salinity. Treat with metronidazole or formalin-based medications. Early treatment significantly improves outcomes.

Prevention: Maintain salinity at appropriate reef levels. Quarantine new fish. Minimize chronic stress.

Bacterial Infections

Bacterial infections in clownfish are usually secondary — they develop at sites of physical injury, parasite damage, or in fish weakened by poor water quality or chronic stress.

Symptoms: Red or inflamed patches on the body; fin deterioration; open sores or ulcers; lethargy; appetite loss.

Cause: Bacterial infection secondary to injury, parasite damage, or poor water quality.

Treatment: Improve water quality immediately. Treat with a broad-spectrum antibacterial medication in a hospital tank. Identify and address the underlying cause.

Prevention: The Most Important Section

The vast majority of clownfish diseases are preventable:

  • Quarantine all new fish for 4–6 weeks before adding them to the display tank — this is the single most effective prevention measure
  • Choose captive-bred clownfish — they arrive with lower parasite loads and are more disease-resistant than wild-caught fish
  • Maintain excellent water quality — ammonia and nitrite at 0 ppm, stable salinity, nitrate under 20 ppm

  • Minimize stress — compatible tankmates, adequate hiding spots, stable parameters
  • Have a hospital tank ready — treating disease in a display reef tank is difficult or impossible with most effective medications
