Blue Tang HLLE

Head and lateral line erosion (HLLE) is one of the most common conditions affecting blue tangs in captivity. It causes progressive pitting, erosion, and discoloration along the head, around the eyes, and along the lateral line. HLLE is not caused by a pathogen and is not contagious, but it is a sign that something in the fish's environment or diet needs to be corrected.

What Does HLLE Look Like?

  • Pitting or cratering along the head and forehead, particularly around the eyes and nostrils
  • Erosion or loss of tissue along the lateral line, which runs from behind the gill plate to the tail
  • Discoloration in affected areas - ranging from pale or washed-out to darker than surrounding tissue
  • In advanced cases, significant tissue loss and a visibly altered appearance along the head and sides

HLLE progresses slowly over weeks to months. Early-stage HLLE may appear as minor pitting or slight discoloration that is easy to miss.

What Causes HLLE?

HLLE is considered multifactorial - no single cause has been definitively established:

Nutritional Deficiency

Inadequate plant matter in the diet is the most commonly cited and most controllable cause. Blue tangs require daily nori and algae-based foods in captivity. Activated Carbon

Long-term use of activated carbon has been associated with HLLE in blue tangs and other tang species. The mechanism is not fully understood. Many experienced reef keepers reduce or eliminate activated carbon use in blue tang tanks, or use it only intermittently rather than continuously.

Poor Water Quality

Stray Voltage

Stray electrical current in the water has been proposed as a contributing factor, as the lateral line is an electroreceptive organ. If stray voltage is suspected, check for it with an appropriate meter. The priority is identifying and replacing any faulty equipment - a leaking heater or powerhead should be found and removed rather than simply adding a grounding probe. A titanium grounding probe may be used as part of the solution after the source has been identified.

Chronic Stress

Chronic stress from an undersized tank, aggressive tankmates, or poor environmental conditions is associated with HLLE. A blue tang kept in a tank that is too small is more likely to develop the condition.

Can HLLE Be Reversed?

Early-stage HLLE can improve significantly - and sometimes fully resolve - with diet correction, water quality improvement, and removal of contributing factors. Advanced HLLE with significant tissue loss is less likely to fully reverse. Improvement is still possible, but complete restoration of the fish's original appearance is not guaranteed. The condition can stabilize and partial improvement in coloration and texture is common.

How to Treat HLLE

  1. Improve diet immediately. Offer daily nori on a veggie clip. Add spirulina flake or frozen spirulina brine shrimp. See our Blue Tang Feeding Guide.
  2. Discontinue or reduce activated carbon use. Switch to intermittent use or eliminate it entirely.
  3. Improve water quality. Increase water change frequency if nitrate is elevated. Test weekly. See our Best Reef Test Kits guide.
  4. Check for stray voltage with an appropriate meter. If detected, identify and replace the faulty equipment first. A titanium grounding probe may be used as part of the solution after the source is found.
  5. Reduce stress. Ensure the tank is 100+ gallons. Remove aggressive tankmates if present.
  6. Be patient. Allow 2-3 months of consistent diet and water quality improvement before assessing progress.

How to Prevent HLLE

  • Feed daily nori from day one - do not wait for symptoms to appear
  • Use activated carbon intermittently rather than continuously, or avoid it in blue tang tanks
  • Maintain excellent water quality with regular testing and water changes
  • Provide a 100+ gallon tank to minimize chronic stress

  • Check for stray voltage periodically, particularly in tanks with multiple electrical devices
