Guppies are hardy fish, but they are susceptible to a range of diseases — particularly when water quality is poor, temperatures are unstable, or new fish are introduced without quarantine. Most guppy diseases are treatable if caught early. This guide covers the most common diseases, how to identify them, and how to treat them.

Ich (White Spot Disease)
Symptoms
- Small white spots on body, fins, and gills — resembling grains of salt
- Flashing (rubbing against decor or substrate)
- Clamped fins, lethargy, reduced appetite in moderate to severe cases
Cause
Ich is caused by the parasite Ichthyophthirius multifiliis. It is introduced through new fish, plants, or equipment and becomes active when fish are stressed by temperature drops or poor water quality. The parasite is only vulnerable to treatment during its free-swimming stage.
Treatment
- Raise temperature gradually to 80–82°F over 24–48 hours to accelerate the parasite’s life cycle
- Increase aeration — warmer water holds less oxygen
- Treat with an ich-specific medication (Ich-X by Hikari is widely recommended)
- Remove activated carbon before treating
- Continue treatment for at least 7 days; perform small water changes every other day and re-dose after each change
Velvet (Oodinium / Gold Dust Disease)
Symptoms
- Fine gold, rust, or yellowish dust on the body — most visible under a flashlight held at an angle in a darkened room
- Flashing, clamped fins, rapid gill movement
- More advanced cases: lethargy, loss of appetite, skin peeling
Cause
Velvet is caused by Oodinium pilularis (or related species), a parasitic dinoflagellate. It is more common in guppies than in many other freshwater species and can spread rapidly through a tank. It is often confused with ich but the particles are finer and have a golden or dusty appearance rather than distinct white spots.
Treatment
- Dim or turn off tank lighting — the parasite is photosynthetic and light supports its life cycle
- Raise temperature gradually to 80–82°F
- Treat with a copper-based medication or a product specifically labeled for velvet (Kordon Rid-Ich Plus, API Super Ich Cure)
- Remove activated carbon before treating
- Treat for the full recommended course — velvet can appear to clear before the life cycle is broken
Fin Rot
Symptoms
- Frayed, ragged, or uneven fin edges
- White, grey, or brown discoloration along fin margins
- Red streaks or inflammation at the fin base in advanced cases
- Fins appearing shorter as tissue is progressively lost
Cause
Fin rot is a bacterial infection caused by opportunistic bacteria (Aeromonas, Pseudomonas, Flavobacterium) that are present in virtually all tanks. It develops when a fish’s immune system is compromised by poor water quality, stress, or physical fin damage. In guppies, fin-nipping tankmates are a common contributing factor.
Treatment
- Test water and perform a 25–50% water change immediately — improving water quality is the most important step
- Remove the cause (fin-nipping tankmates, overcrowding)
- For mild cases, water quality improvement alone may be sufficient
- For moderate to severe cases, treat with an antibacterial medication (Kanaplex by Seachem is commonly used for gram-negative bacterial infections)
- Continue water changes throughout treatment and re-dose as directed
Columnaris
Symptoms
- White or grey patches on the body, often near the dorsal fin (saddle mark) or mouth
- Frayed fins with white edges that may look cottony
- Lesions can progress rapidly — columnaris can kill fish within days in acute cases
- Gills may appear pale or discolored in severe cases
Cause
Columnaris is caused by the bacterium Flavobacterium columnare. It is often introduced through new fish and becomes active under stress. It can be confused with fungal infections but is bacterial in origin and requires antibacterial rather than antifungal treatment.
Treatment
- Improve water quality immediately
- Treat with an antibacterial medication effective against gram-negative bacteria — Kanaplex or a combination of Kanaplex and Furan-2 is commonly used for columnaris
- Act quickly — columnaris progresses faster than most bacterial infections
- Quarantine affected fish if possible to prevent spread
Wasting Disease (Fish TB / Mycobacteriosis)
Symptoms
- Progressive weight loss despite eating normally
- Curved or bent spine (spinal deformity)
- Lethargy, loss of color, sunken abdomen
- Chronic, slow decline over weeks or months
Cause
Wasting disease in guppies is most commonly associated with Mycobacterium species (fish tuberculosis) or internal parasites. Mycobacteriosis is difficult to diagnose definitively without laboratory testing and is essentially untreatable in most aquarium settings. It can be contagious to other fish and, rarely, can cause skin infections in humans with open wounds — use gloves when handling water from a tank with suspected mycobacteriosis.
Management
There is no reliable cure for mycobacteriosis. Affected fish should be humanely euthanized to prevent spread. The tank should be thoroughly cleaned and disinfected before restocking. Internal parasites causing wasting can sometimes be treated with antiparasitic medications such as Levamisole or Fenbendazole, but diagnosis is difficult without ruling out mycobacteria first.
Dropsy
Symptoms
- Swollen, bloated abdomen
- Scales raised and protruding outward (pinecone appearance when viewed from above)
- Lethargy, loss of appetite
Cause
Dropsy is not a disease itself but a symptom of organ failure — usually kidney failure — caused by bacterial infection, viral infection, or other systemic illness. By the time pineconing is visible, internal damage is typically severe. Treatment success rates are low.
Treatment
Isolate the affected fish immediately. Epsom salt baths (1 tablespoon per gallon) can help reduce fluid retention temporarily. Antibacterial treatment with Kanaplex may help if the underlying cause is bacterial and caught very early. Euthanasia is often the most humane option once full pineconing is present.
General Prevention
- Quarantine all new fish for 2–4 weeks before adding them to an established tank
- Maintain water quality with weekly testing and regular water changes
- Keep temperature stable — sudden drops trigger ich and velvet outbreaks
- Avoid overcrowding and fin-nipping tankmates
- Feed a varied, high-quality diet to support immune function
- Remove dead fish immediately to prevent water quality decline and disease spread
