Angelfish Diseases & Health

Angelfish are hardy fish when kept in well-maintained aquariums, but they are susceptible to the same diseases that affect most freshwater tropical fish. The majority of health problems in angelfish are directly linked to poor water quality, stress, or inadequate diet — meaning most are preventable with good husbandry. This guide covers the most common angelfish diseases, how to identify them, and how to treat them.

Prevention First

Most angelfish disease outbreaks are triggered by one or more of the following:

  • Poor water quality — elevated ammonia, nitrite, or nitrate
  • Temperature fluctuations or incorrect temperature
  • Stress from overcrowding, aggression, or incompatible tank mates
  • Introduction of new fish or plants without quarantine
  • Nutritional deficiencies from a monotonous diet

Whenever possible, treat sick angelfish in a separate hospital tank. This protects the biological filter in the display aquarium, reduces stress on healthy fish, and allows medications to be dosed accurately.

Common Angelfish Diseases

Ich (White Spot Disease)

Symptoms: Small white spots resembling grains of salt on the body, fins, and gills. Affected fish may flash (rub against surfaces) and show labored breathing if gills are involved.

Cause: The parasite Ichthyophthirius multifiliis. Highly contagious — spreads rapidly through the tank. Often triggered by temperature drops or stress.

Treatment: Gradually raise water temperature to 86°F (30°C) over 48 hours to accelerate the parasite's life cycle, and treat the entire tank with an ich-specific medication. Remove activated carbon before adding medication. Continue treatment for the full recommended course even after spots disappear. Perform a water change before adding medication.

Hole in the Head (HITH / Head and Lateral Line Erosion)

Symptoms: Pitting, erosion, or mucus-filled cavities around the head, eyes, and along the lateral line. May progress slowly over weeks or months.

Cause: Associated with poor water quality, prolonged use of activated carbon, nutritional deficiencies (particularly vitamin C and D), and in some cases the flagellate parasite Hexamita. Exact causation is debated in the hobby.

Treatment: Improve water quality with increased water change frequency. Remove activated carbon from the filter. Improve diet with varied, vitamin-rich foods. If Hexamita is suspected, treat with metronidazole. Early cases often resolve with husbandry improvements alone.

Fin Rot

Symptoms: Ragged, fraying, or receding fin edges. May appear white or red at the margins. In severe cases, the fin tissue deteriorates back toward the body.

Cause: Bacterial infection, typically secondary to poor water quality, physical injury, or fin-nipping tank mates. Stress lowers immune response and makes fish more susceptible.

Treatment: Improve water quality immediately with a water change. Remove any fin-nipping tank mates. Treat with an antibacterial medication if the condition is progressing. Remove activated carbon before medicating. Mild cases often resolve with water quality improvement alone.

Velvet (Gold Dust Disease)

Symptoms: Fine gold, rust, or yellowish dust on the body and fins, most visible under a flashlight at an angle. Affected fish may flash, clamp fins, and show rapid gill movement.

Cause: The dinoflagellate parasite Oodinium. Highly contagious and can be fatal if untreated.

Treatment: Dim or turn off aquarium lights (the parasite is photosynthetic). Raise temperature gradually to 82–84°F. Remove activated carbon from the filter. Treat the entire tank with a copper-based medication or formalin treatment.

Internal Parasites

Symptoms: Wasting despite a good appetite, stringy white or clear feces, bloating, or hollow belly. Fish may become lethargic over time.

Cause: Internal parasitic infections — more common in wild-caught angelfish than captive-bred specimens. Can also be introduced via live foods collected from outdoor sources.

Treatment: Treat with an antiparasitic medication containing praziquantel or levamisole for worms, or metronidazole for flagellate infections. Remove activated carbon before medicating. A fecal sample examined by a vet can confirm the type of parasite if the cause is unclear.

Bacterial Infections (Columnaris / Mouth Rot)

Symptoms: White or grey patches on the body, mouth, or fins. May resemble a fungal infection. Affected tissue may erode. Fish may stop eating.

Cause: The bacterium Flavobacterium columnare, often triggered by stress, injury, or poor water quality.

Treatment: Improve water quality. Remove activated carbon. Treat with an antibacterial medication. Columnaris can progress rapidly — begin treatment promptly if symptoms are identified.

Dropsy

Symptoms: Severe bloating with scales raised and protruding outward (pinecone appearance). Often accompanied by lethargy and loss of appetite.

Cause: Dropsy is a symptom of organ failure — typically kidney failure — rather than a disease itself. It can be caused by bacterial infection, viral infection, or severe parasitic infestation.

Treatment: Prognosis is poor once pineconing is visible. Isolate the affected fish in a hospital tank. Treat with antibiotics and Epsom salt baths. Consult a vet if possible. Euthanasia may be the most humane option in advanced cases.

A Note on Activated Carbon

Remove activated carbon before using most aquarium medications, as carbon can absorb medication and reduce its effectiveness. Replace fresh carbon after treatment is complete if desired.

Quarantine

Always quarantine new fish for a minimum of 2–4 weeks in a separate tank before introducing them to an established aquarium. Many diseases are introduced by apparently healthy fish that are carrying pathogens without showing symptoms. A simple quarantine tank — a bare 10–20 gallon tank with a sponge filter and heater — is one of the most effective disease prevention tools available.

When to Seek Veterinary Help

Most common angelfish diseases can be treated at home with commercially available medications. However, if a fish is not responding to treatment after the full recommended course, if multiple fish are dying rapidly, or if you are unsure of the diagnosis, consult an aquatic veterinarian. Misidentifying a disease and using the wrong medication can make the situation worse.
